Please find attached with this letter the Freedom of Information request you made on behalf of [text removed] relating to the subject of NHS continuing healthcare.

We can confirm that NHS Calderdale CCG holds some of the information described in your request as stated below.

Section 12 (1) of the Freedom of Information Act makes provision for public authorities to refuse requests for information where the cost of dealing with them would exceed the appropriate limit. The appropriate limit has been specified in the Freedom of Information and Data Protection (Appropriate Limit and Fees) Regulations 2004. For public authorities the appropriate limit is set at £450. This represents the cost of one person spending 2.5 working days in determining whether NHS Calderdale CCG holds the information, locating, retrieving and extracting the information.

We estimate that it will cost more than the appropriate limit to fulfill the whole of your request. This is because the way that information regarding Continuing Healthcare is stored does not enable us to readily extract or collate the specific details you are requesting as detailed in relation to a number of the questions you raise.

The majority of the records prior to 2016 are manual records which would need to be retrieved from storage and each patient file would need to be read to extract the specific information you are requesting. For the records that are held electronically, each file would need to be accessed and read to extract the information you request.

There were in the region of 400 applications for CHC funding processed by the CCG from June 2015 to March 2016. Therefore providing the information you are requesting for 3 financial years would involve retrieval and scrutiny of at least 1,000 files. Once the manual records had been retrieved we estimate it will take over 80 hours of staff time to extract and collate the information (based on 5 minutes per record). We are therefore not obliged under Section 12(1) of the Freedom of Information Act to fully respond to your request in its entirety. We have however set out below against each question what we believe we can provide within the appropriate limit.

However we are able to provide;
• 1c – Community information for the period June 2015 to March 2016;
Hospital information for the period August 2015 to March 2016.

• 4a and 4b for years 2013/14 and 2014/15 up to June/August 2015;

However we are able to provide;
• 4c – Community information for the period June 2015 to March 2016;
Hospital information for the period August 2015 to March 2016.

4. Does your CCG always involve experts in the assessment of the specific medical condition that the person being assessed for NHS continuing healthcare presents with? For example a Parkinson’s nurse, an MS specialist nurse, a neurologist etc.

NHS Calderdale would be able to provide the response in its entirety;

5.1.We know that people with long term, progressive conditions who are found eligible for NHS continuing healthcare are often reassessed after a set period.

Please supply the total number of NHS continuing healthcare reviews conducted by your CCG, on people who have existing eligibility. Please include all reviews including 3 month and annual.

NHS Calderdale CCG is unable to provide the following information within the appropriate time limit;
• 6a and 6b for years 2013/14 and 2014/15;

However we are able to provide;
• 6c – NHS Calderdale would be able to provide the response in its entirety.

5.2.The total number of cases in your CCG where NHS continuing healthcare eligibility was withdrawn following the review of a person previously found eligible for NHS continuing healthcare.

NHS Calderdale CCG does not hold this information.
