Better Care Fund (151679)

The information I am requesting relates to the Better Care Fund. Given that Health and Wellbeing Boards are not separate public bodies for Freedom of Information purposes, I am directing this request to CCGs.
Under the 2015-16 Better Care Fund policy framework, local areas were asked to set targets against five key metrics:

• Admissions to residential and care homes
• Effectiveness of reablement
• Delayed transfers of care
• Patient / service user experience
• A locally-proposed metric

I am requesting the following information:

1. The CCG’s locally-proposed metric.

2. The targets set by the CCG (or HWB in which the CCG participates) for each metric – please state precise statistical targets where applicable.

Please state the “starting position” for each metric – i.e. please give statistics on how that metric was performing at the start of the measuring period (e.g. if the Delayed Transfers of Care target is to reduce DTOCs from their 2014/15 level, please state the DTOCs figure for 2014/15).

There is a joint team between the council and the CCG which work on producing and monitoring the performance information. The equivalent report to this goes to the CCG’s Finance and Performance Committee

Section B of the main report gives a brief narrative overview of the position on each of the metrics and appendix A shows the baseline and target and actual figures as requested in the FOI request

3. Please state whether the CCG or the local authority (please state which) is responsible for measuring progress for each target.

The up to date BCF quarterly report paper was presented to the Health and Wellbeing Board on the 14th January and can be found on the Council website via the attached link. This is a publically available document.

4. Please state what statistical progress to date has been made towards each target, and whether each target is currently forecast to be achieved by the end of 2015/16.

The CCG does not forecast therefore this information is not available.
