Burley Street Surgery will soon be closing

Dr Falak Naz’s contract to provide GP services at the surgery will shortly come to an end.  This means that Dr Naz’s practice will close at 6.30 pm on Friday 29 June 2018.

NHS Calderdale Clinical Commissioning Group (CCG) is responsible for the planning and buying of GP services in Calderdale.  We want to make sure that you will have a GP after Dr Naz retires.

What do you need to do now?

You will need to register with a new GP practice in your local area.

We understand that you might find it difficult or be worried about having to change doctors, so we will make sure there is lots of support to help you.

Drop-in sessions

We have also arranged two drop-in sessions where staff will be able to answer any questions you may have about this change and support you to find a new GP Practice.

These drop-in support sessions will be held at:  Bertie’s, Brook Street, Elland HX5 9AW on the following dates:

    • Wednesday 6th June 2018 at 6pm to 8pm, and
    • Friday 15th June 2018 at 10am to 12 noon

Frequently Asked Questions

Why do I have to be seen by a different GP before the closure date?

Dr Falak Naz has left the practice, but has ensured that there is another doctor at the practice until the it closes at 6.30 pm, Friday 29th June 2018.

This is to allow you time to register with a new practice and enable you to keep any pre-booked appointments.

Will my appointment times and dates be the same until the practice's closure date?

All appointments that have been already booked at the Burley Street surgery will be covered by another doctor, so you will not need to book another appointment. Please attend your appointment as normal.

Will the other doctor know what medicines I'm taking/what treatment I'm on?

Don’t worry; the doctor will have access to your medical records so that they can use them as part of your consultation. It is always handy to make a list of the medicines you are taking and bring it with you to your appointment.

Why do i have to register with a new, locfal GP practice?

As Dr Naz is the only GP at Burley Street Surgery the practice will close at 6.30pm on Friday 29th June 2018, this means you can no longer be seen at Burley Street and you will need to register with new GP practice.

I’m pregnant / have a long-term condition / am in the middle of a course of treatment?

In the coming weeks, we will work with Burley Street Surgery to make sure that patients who are in the middle of a course of treatment/ are pregnant/ have a long-term condition are provided with the support they need to find a suitable new practice.

My prescription is due in the next few weeks – should I move practice immediately?

If you’re expecting a new repeat prescription in the next few weeks, we’d recommend you change your GP practice once you have collected it.

You need to have registered with a new GP practice by 8th June 2018.

I’m worried that a new GP might change my medication.

As part of the GP registration process, some practices will undertake a standard health check, including a review of any medication you are currently taking.

If this is not the case with your new GP, you should arrange to have a check-up as soon as possible to discuss any medication you’re taking.

I am worried about my care in the future.

General Access

Inform guests to use main door, the Crystal Ballroom is located on the first floor

Wheelchair Access

There is a door located to the left of the main door (staff door) and this takes guests to the lift which will take them to them directly into the Crystal Ballroom.

Parking

Brook Street car park is next to Bertie's, it is owned by Calderdale Council so there is a charge of 40p per hour up until 6pm.

I’m worried that a new GP might not have the translation/interpreter service I use now.

You will continue to be able to access the same translation/interpreter services at all Calderdale GP practices.

Will I be left without a GP practice?

No, the CCG is responsible for ensuring that you continue to receive GP services.  All patients need to register with a new local GP practice before Friday 8th June 2018. If you haven’t registered with a new GP practice by then we will do this for you.  This means that you will be put onto a list of a local GP and your medical records from Burley Street Surgery will transfer automatically to this new practice.

How do I know which practice to choose?

You can also use the NHS Choices website at www.nhs.uk to find local practices. The website has a range of information about practices as well as patient feedback.

It is a good idea to visit the practice website, or call into the surgery to discuss the services available and find out if they meet your needs.

Things that may help you choose a new practice could include how many GPs there are at the surgery, the opening hours, what services they offer, how they have been rated by the Care Quality Commission, and any patient feedback.

Other GP practices are too far away for me to get to. What am I going to do now?

We know that there are at least eleven nearby practices accepting new patients which range from half a mile to two and a half miles from Burley Street Surgery (this list can be found under the which other GP practices can I join nearby header above).

I have been with this practice for some years and I’m worried about having a new doctor.

We do understand that it can be a worrying time for you.  Please be assured that we are committed to making sure that you get settled with a new practice.  All GP practices and their staff have to meet high quality standards and the Care Quality Commission regularly inspects and monitors practices to ensure these standards are achieved.

How are you communicating information about the closure?

Every patient registered at the Burley Street Surgery has been sent a letter giving more information about the changes and what you need to do next. These should start arriving with patients from Monday 21st May.  We’re also working with the practice to communicate the changes and provide information within the surgery too.

What will happen to the staff at the practice?
