We are delighted to announce that Calderdale has become one of 29 “vanguard” sites in England to take a national lead in transforming health and social care as part of NHS England’s New Models of Care programme.
The Calderdale health and social care partnership has been chosen from amongst 269 groups of nurses, doctors and other health and social care staff from across the country who put forward their ideas for how they want to redesign care in their areas. The partners are:
- NHS Calderdale Clinical Commissioning Group
- Calderdale Council
- Calderdale and Huddersfield NHS Foundation Trust
- GP Pennine Alliance
- South West Yorkshire NHS Foundation Trust
- Voluntary Action Calderdale (represents over 300 health-related 3rd sector organisations)
The 29 winning bids were chosen as the most innovative plans for transforming patient care and will be able to draw on a new £200 million transformation fund and tailored national support. From April the vanguards will develop local health and care services to keep people well, and bring home care, mental health and community nursing, GP services and hospitals together for the first time since 1948. For patients, this will lead to a significant improve in their experience of health services.
