The Calderdale Formulary has been developed to support cost effective prescribing in primary care. It is not intended to be fully inclusive but to cover common prescribing situations. The clinical freedom of prescribers to choose the most appropriate treatment for their patients is not restricted by this formulary. To support ease of prescribing standard instructions have been added for many medicines and, where appropriate, wording to support safer prescribing has been added including allergy information where identified, please note: this is not an exhaustive list.
