In Calderdale we aim to support people to live in their own home for as long as they can, but if this is no longer possible then we must ensure that the best possible care is provided to those people who live in residential settings. We want to make sure that care homes are just that: a home, and that people are valued and treated as individuals.
To ensure we create better services we need the support of local people in the community. We want views and ideas from local people will help us shape the model of care provision in Calderdale for the future.
You have until Friday 28 April to give us your thoughts on the future of care home provision in Calderdale, either by clicking the button above or by printing this questionnaire and returning to the freepost address given at the end of the document.
Your views will be included in the work Calderdale CCG, along with Calderdale Council, is doing to develop and improve services for older people who require 24 hour care.
