Specialist medical input (e.g. medical consultants with expertise in life-shortening conditions) (yes/no)?
As part of Paediatric services that we commission, provision for life-shortening conditions is included. We also commission End of Life services which have specialist medical input which also cater for those under 25 years but will liaise with the appropriate tertiary centre that are commissioned by NHS England.
Community children’s nursing (yes/no)?
NHS Calderdale CCG commissions community children’s nursing.
If the child has a full NHS Continuing Health Care or Fast Track service in place, then we would provide overnight services.
For children who are not under c/o Community Children’s Continuing Health Care team, we have an agency in place.
There are several children in Calderdale who are in receipt of this service. There is a long term ventilation pathway which recommends overnight care which we follow.
Children’s hospice services (yes/no)?
We commission Martin House which provides care out of hours and at weekends.
Community paediatrics (yes/no)?
The CCG commissions this service from Calderdale and Huddersfield Foundation Trust The service includes care out of hours and at weekends.
Specialised children’s palliative care (which can include managing complex symptoms and prescribing unlicensed medicines) (yes/no)?
The CCG commissions children’s palliative care as part of community paediatric service provided by Calderdale and Huddersfield Foundation Trust.
The CCG also commissions a Palliative Care Out of Hours Service for people aged 18 – 25 years. This service is provided by Calderdale and Huddersfield Foundation Trust as the Lead provider, in conjunction with Overgate Hospice and Marie Curie.

Emotional and psychological support – including bereavement care (yes/no)?
The CCG commissions a Children and Adults Mental Health Services (CAMHS) Tier 2 and Tier 3 which provides support to children and young people depending on their needs. Services are generally provided in office hours although there is crisis support available 24/7.
Equipment – including wheelchairs (yes/no)?
There is provision in the case of emergencies for equipment including wheelchairs to be provided out of hours and at weekends.
