NHS Calderdale Clinical Commissioning Group (CCG) is responsible for planning and buying healthcare services for the local population.

In July we wrote to all households with people registered at one of our Alternative Provider Medical Services (APMS) GP practices, asking what they thought about some changes we wanted to make.

We have looked at what people told us and thought about what we can do. We now want to consult you on our plan to change these services.

It is really important that we understand the impact our plan may have on you and your family so we can take your views into account when we make our decisions, so please take the time to complete our survey.
